Early Accomplishments


Plan Evaluation

The purpose of evaluating the Minnesota Diabetes Plan 2010 is to determine how progress is being made on the Plan's recommendations, to improve Plan marketing/implementation and to inform future strategic planning for diabetes in Minnesota.

The Plan evaluation will assess:

  • Plan marketing and promotion
  • Plan use and action on recommendations (implementation)
  • Progress on Plan recommendations (achievements & outcomes)
  • Progress on diabetes public health goals

A Plan Evaluation Advisory Group (PEAG) was convened in November 2004 to provide expertise and diverse perspectives to the Plan's evaluation. The PEAG includes staff of the Minnesota Diabetes Program, evaluators of other statewide chronic disease plans, and local evaluation experts.

A "working draft" of the Minnesota Diabetes Plan 2010 Evaluation Design is now available. Your comments on this document are welcome.

The Minnesota Diabetes Plan 2010 Logic Model illustrates how Plan inputs and activities are designed to produce short-term, mid-term and long-term outcomes.

Progress Reports

The purpose of these reports is to keep Minnesota Diabetes Plan 2010 stakeholders, and other members of the Minnesota diabetes community, up-to-date on the progress being made on the Plan. Specifically, Plan Progress Reports will showcase examples of Plan-related strategies and innovations, and identify gaps where more action is needed. Progress Reports will be released annually.

The reports will be compiled by the Minnesota Diabetes Program (MDP) with assistance from the Minnesota Diabetes Steering Committee, and funding from the CDC. As a public health program, one of the roles of the MDP is to monitor and report on progress toward achieving Plan goals.

You can help play an important role in monitoring activities and evaluating the impact of the Plan. If you are aware of other key milestones or achievements, please contact us or enter them into the Action Network on Diabetes Plan CENTRAL. We will highlight these accomplishments on the Plan CENTRAL website and in future publications.
