The Minnesota Diabetes Plan 2010 (the Plan)
is the second statewide 10-year strategic plan for diabetes in the
state of Minnesota. The purpose of the Plan is to communicate
the vision of creating a healthier future for all people in Minnesota
and provide a roadmap for achieving that vision. The goals and recommendations
of the Plan show our community how to work together more
effectively to reduce the burden of diabetes in Minnesota.
Diabetes Plan CENTRAL (Collaborative Exchange Network To Raise
Awareness & Learning) is a web tool designed to serve as an
interactive communication hub for the diabetes community to facilitate
accomplishing the goals of the Plan.
