WHAT DOES IT MEAN TO “PARTICIPATE”?
Welcome to Participate! This section of Diabetes Plan CENTRAL is
intended to give you the opportunity to share the good work you
are doing and the progress you are making toward achieving the goals
and recommendations of the Minnesota Diabetes Plan 2010. Participate
is also designed to give the Minnesota diabetes community a way
to learn about the good work that colleagues from other sectors
and regions of the state are doing.
Features of Participate include searching the Plan goals and recommendations,
registering to endorse the Plan, sharing information about programs
and activities, sharing tools and other resources and eventually,
sharing success stories and lessons learned in working to achieve
the goals of the Plan.
It is our hope that Participate will not only help facilitate communication
about the Plan, but will serve as a well-spring for new ideas, new
partnerships and ongoing inspiration to help realize the overall
vision of the Plan; creating a healthier future for all people in
Minnesota.
Search the Plan Goals:
If you are unsure what the Plan encompasses; if you want to know
whether or not your organization’s programs and activities
help address the goals the Plan; or if you just want to get new
ideas for how to reduce the burden of diabetes in Minnesota Search

electronically search the goals and recommendations of the Minnesota

Plan Champions:
What does it mean to endorse the Plan? Endorsing the Plan means
that you, as an individual or an organization, wish to publicly
indicate your support for the vision and goals of the Minnesota
Diabetes Plan 2010.
By endorsing the Plan, you are registering to become a Diabetes
Plan Champion who will be acknowledged on the Diabetes Plan CENTRAL
website and in Plan-related promotional materials. Registering as
a Champion ensures that you will receive up-to-date information
on everything from Plan-related activities to outcomes. It also
means that we will periodically request your feedback on various
aspects of the implementation and evaluation of the Plan.
To enable us to communicate effectively with Diabetes Plan Champions,
we request that you provide and maintain current contact information
with us.

Action Network:
The Action Network has been designed as a way for the Minnesota
diabetes community to share information on past, current or future-planned
programs and activities that help to achieve the goals the Plan.
The Action Network is more than a repository of information; it
is intended to serve as a source of inspiration and momentum for
new projects, new partnerships and new resources.
If you or your organization are currently leading or planning a
diabetes initiative, you are encouraged to join the Action Network,
and share information about your program and activities. If you
are simply interested in learning more about the many ongoing initiatives
addressing the Plan, you are invited to browse the Action Network.
Please note, if you choose to participate in the Action Network,
the information you enter about your program or activities will
be accessible to anyone with access to the Internet. However, you
can choose not to share personal contact information.

Resource List :
The Plan Resource listing is one way for the Minnesota diabetes
community to share tools, including manuals, journal articles, presentations,
maps, reports, survey instruments, software, websites and other
resources with the rest of the community.
Any individual or organization may add a resource to the listing
that they have used, developed or would like to recommend to others
working to achieve the goals of the Plan. Please note, when you
add a resource to the listing, the information you enter will be
accessible to anyone with internet access. However, you can choose
not to share personal contact information.
