Description: Please join us from noon - 1:00 pm for a webinar presented by Katherine Mackedanz, Todd County Health & Human Services, and Julia R. Draxten, CentraCare Long Prairie Clinic. CentraCare Health-Long Prairie and Todd County Health & Human Services collaborated on a Learning Community project that was data driven and emphasized their ongoing collaboration. Through the data review process, it was discovered that tobacco use rates were disproportionately high in the PMAP (Prepaid Medical Assistance Program) population. Staff worked together to identify ways to promote CentraCares Tobacco Cessation program, create outreach materials and engage community partners to identify new methods to reach the intended population.
