Position: Nurse Practitioner

Description: NorthPoint Health and Wellness Center is seeking a Family Nurse Practitioner to provide a full-spectrum of ambulatory family medicine in a community clinic setting. Our practice is a diverse, urban, community health center whose mission is "Partnering to create a healthier community." Our main campus is located in North Minneapolis. NorthPoint also supports off site and school based clinics. Work hours for this position will be part-time, benefit earning, Monday through Friday. Occasional coverage for evening or Saturday clinic hours may be requested. The primary duties and responsibilities of this position include: "Obtain health history, perform physical examination and conduct preventive screening. "Evaluate, diagnose and treat acute and chronic illness and disease. "Provide preventive services for patients of all ages. "Make appropriate referrals to other health professionals and community agencies. "Participate in NorthPoint's quality management program. "Utilize electronic health records. "Design, implement, and evaluate programs of care based on the needs of our population. "Triage those who require physician supervision; work as part of the care team. "Partner with patients and families to optimize their health and wellness.
Required Qualifications: "A master's degree or higher in nursing from an accredited program. "Two or more years of full-time experience as a family nurse practitioner in a community health clinic or family practice environment. "Licensure as a registered nurse by the state of Minnesota. "Certification as a Family Nurse Practitioner or an approved equivalent as provided under M.S. 148/284. "Knowledge of adolescent health and school based clinics. "Fluency in Spanish or Hmong, or experience working with interpreters preferred. "The ability to respect, appreciate and promote teamwork among diverse co-workers and patients in regard to race, age, gender, disability, values and lifestyle perspectives or interests.
