When submitting your story please provide the following information: your name, affiliation, email, phone number and the story narrative. Email this information and your story to health.stories@state.mn.us
Informed Consent
Stories are submitted voluntarily to MDH. MDH assumes that, when a stories is submitted for use on the MDH stories web site, the author gives permission for the story to be used for education, training and other related purpose. MDH also assumes that the author understands that the decision whether or not to post the story on the web site will be made by staff of MDH; and that if posted, the story may be edited before being posted. In addition, MDH assumes that there is no personal or identifying information included in the story. MDH does not have any responsibility for coincidences or similarities to actual circumstances of a real life situation.
