Continuing Education Certificates and
Conference Evaluation

Thank you for attending the 2012 Community Health Conference.

CEU certificates are available for dieticians, environmental health sanitarians/specialists, health educators, nurses, and general attendees.

In order to receive a CEU certificate, you must first fill out a conference evaluation.

If you attended the conference, you should have received an invitation to fill out an evaluation survey during the week of October 8-12, 2012; if you would like your survey link re-sent to you, please contact: health.chconference@state.mn.us
