  • Mobilizing for Action through Planning and Partnerships (MAPP)

    MAPP is a community-driven strategic planning tool for improving community health. Facilitated by public health leaders, this tool helps communities apply strategic thinking to prioritize public health issues and identify resources to address them. MAPP is not an agency-focused assessment tool; rather, it is an interactive process that can improve the efficiency, effectiveness, and ultimately the performance of local public health systems.
  • Association for Community Health Improvement (ACHI)

    ACHI is the premier national association for community health, healthy communities and community benefit. They convene and support leaders from the health care, public health, community and philanthropic sectors to help achieve shared community health goals. You will find tools for planning, assessment and evaluation under "Resources" on this site.

    A practical guide to planning, leading and using community health assessments, developed by the Association for Community Health Improvement.
