  • Positive Alternatives 2012-16 Grant Application
    This page provides the forms and instructions you will need to complete the Positive Alternatives Grant Program application.

  • Direct Deposit Form (PDF document: 395KB/2 pages)
    The Minnesota Dept. of Finance requires that all payments to grantees be made by direct deposit from the state to a financial institution designated by the grantee. Use this form to tell the Dept. of Finance where to send your invoice payments.

  • Baby Cry Cards
    "It's not easy taking care of a baby. Babies can be fussy and cry a lot." is the message of these colorful 5.5" x 8.5" cards. This Shaken Baby Syndrome Prevention resource is available in Hmong, Somali and Spanish.   • Woman's Right To Know Materials
    The Women's Right to Know booklet "If you are pregnant: Information on fetal development, abortion and alternatives" is a valuable resource for Positive Alternatives grantees. It contains the Commissioner of Health information referred to in the Positive Alternatives statute. The booklet is available in English and Spanish.

  • Smoking Tear Sheets
    A tool for prenatal care providers to use when offering support and advice to pregnant women to encourage smoking cessation or reduction. This 2 page guide lists "Keys for Quitting" and steps to set up "Your Quit Plan".

  • Developmental Wheel
    The Developmental Wheel describes some of the development children have by the time they reach certain ages (birth to five), as well as related nutrition, hearing, speech and language, and vision information. Copies of the English, Spanish, Hmong, or Somali Developmental Wheels are available free to Minnesota families and organizations.
