Pediatric Care Coordination Request for Proposals
The Minnesota Department of Health is seeking proposals from nonprofit organizations, Institutes of Higher Education, clinics or hospitals that will manage and implement the care coordination project.
The goal of this project is to test, evaluate and plan for the spread of successful care coordination model(s) for Children and Youth with Special Health Needs (CYSHN). The overall objectives of implementing successful care coordination models are to decrease fragmentation, coordinate care across specialty as well as community resources, and increase positive outcomes across the lifespan for CYSHN and their families. The successful applicant will recruit a minimum of four primary care clinics. The applicant will provide technical assistance to the primary care clinics who will implement the model(s) of pediatric care coordination to be tested and evaluated, working with staff from MDH as needed. The project must include a statewide needs assessment to identify and prioritize the needs in the provision of health care coordination to CYSHN. Families of CYSHN must be included as partners in the decision making throughout the grant’s execution.

Total Funds Available:The total amount of funding for this project is up to $480,000 for the three year grant period. One or more applicants will be funded up to a total of $480,000 for the grant period.
