Notice of Intent Form
Applicants were strongly encouraged to submit a non-binding notice of intent by October 5, 2018 using an online form. The notices of intent to apply help us prepare for the number and types of applications we will receive so that we can plan ahead for the review process. The Center for Health Equity received 164 Notice of Intent forms by October 5, 2018.
