Changing (Amending) a Death Record

If there is incorrect or missing information on a death record, you may make a request for an amendment. The requirements and options established in Minnesota Rules, part 4601.2100, for amending a death record depend on when the death occurred.

If the death occurred within the last year, you have four options for requesting an amendment to the death record.

1) Through the funeral home

2) Through a local registrar

3) Through the state registrar

4) Through the informant

If the death occurred more than one year ago, but less than five years ago, you have two options for requesting an amendment to the death record.

1) Through a local registrar

2) Through the state registrar

If the death occurred more than five years ago, you have one option for requesting an amendment to the death record.

1) Through the state registrar
