To add the registrant's name:
Submit one of the following documents: an authenticated hospital or clinic record,
an authenticated school record, or an original or certified copy of military
discharge.
To change, or correct the spelling of the registrant's name:
Submit two of the following documents: an authenticated hospital or clinic record,
an authenticated school record, an original or certified copy of military discharge.
To correct the spelling of a parent's name or to correct a parent's birth place
or birth date:
Submit two of the following documents: an authenticated hospital or clinic record,
an authenticated school record, a valid U. S. passport, a certified copy of
the parent's marriage certificate, an original or certified copy of a military
discharge, a certified copy of the parent's birth certificate.

Court Orders
If you submit a certified copy of the court order that specifically directs
the birth record to be changed, you do not need to submit any other supporting
document. The name on the court order must match the name on the birth record
exactly and changes to the birth record are limited to changes that are directed
by the court order.
If you submit a certified copy of a court order that does
NOT direct the birth record to be changed, the court order must meet the age
requirements and you must submit one additional document to support the amendment.
