Credit Card Authorization and Disclaimer
To give you the option to fax your request and pay with a credit card, MDH provides access to the US Bank. By choosing to pay with a credit card you are choosing the US Bank service and you are authorizing MDH to provide information to US Bank to process your credit card request. The information provided to US Bank includes: your name, address, phone number, credit card number and expiration date.
