a- If a funeral home is not a MN VRV user, the funeral home completes the demographic or non-medical part of the worksheet. The worksheet is then sent to the physician to collect the medical or cause of death information. The worksheet is returned to the funeral home for review and is then sent to the local registrar, by the funeral home, for entry into MN VRV.

b- If a funeral home is a MN VRV user, after the cause of death information is collected and the funeral home has reviewed the worksheet, the worksheet is sent to the OSR and the medical or cause of death information is entered into MN VRV.

c- If both the funeral home and the physician are users, the worksheet is only for internal funeral home use, it does not have to be sent to either the local registrar or to the OSR. The physician is able to access the record and complete the cause of death in the computer.

Establishments that are registering death records electronically through MN VRV2000 are not required to file this report.

Minnesota Rules, part 4601.1900, require the person in charge of the funeral establishment or that person's authorized designee to submit this report. The report is due on or before the tenth day of each month and must include a report of all deaths during the previous month for which a mortician or funeral director affiliated with the funeral establishment was responsible for collecting and providing documentation of death according to part 4601.1600. Please follow the instructions on the form for completion and mailing or faxing.

Funeral Director User ID Application

a- The "Funeral Director" User ID application allows the funeral establishment owner/manager to authorize individual funeral directors to have access to the MN VRV2000.

b- The "Funeral Director Staff Assistant" User ID application allows the funeral directors to authorize a staff assistant to have access to the MN VRV2000 to work on their behalf.

c- When adding a funeral director and a staff assistant at the same time, both forms must be used.

d- With the exception of deleting a user, whenever either or both of the forms are used, all of the requested information is required.
