Online Verification of Social Security Number
Minnesota participates in the Social Security Administration's online verification
of social security number (OVS). Through OVS, a decedent's social security
number (SSN) reported through death registration is transmitted to the Social
Security Administration (SSA) to be electronically verified. Funeral directors
receive notification through MN VRV2000 about the accuracy of the SSN as compared
with the name, sex, and date of birth reported on the death record.
Through MN VRV2000, fact of death is transmitted to SSA within one day (24 hours) of the date the fact of death record was filed (i.e., saved).
The following links will provide you with information about OVS including data practices requirements, what funeral directors and funeral home staff need to do to participate, faxing SSA's 721 form, and a link to the December 2005 fact sheet.
What do funeral directors and other funeral home staff need to do to participate?
