Issuance of Multiple Birth Certificates Report (Posted: 1/17/12)
During the 2011 Vital Records Training, we asked you to report to our Fraud Prevention Coordinator and OSR Issuance Supervisor, Brenda Shinaul, each instance of issuing more than three (3) birth certificates for one record during a single transaction. In order to consistently collect the same information for each request, we have added a form to the Forms page to be used for this purpose.
