To apply for asbestos worker, asbestos site supervisor, asbestos inspector, asbestos project designer or asbestos management planner certifications, the following forms must be completed and submitted to MDH:

If you are applying for your asbestos contractors license, the following form must be completed and submitted to MDH:

If you are a licensed contractor and are conducting regulated asbestos-related work, the following form must be submitted to MDH five (5) calendar days before your project begins:

Once your training course has been approved by MDH, you must notify MDH 14 calendar days in advance of the course with this form:
