The following form must be completed and submitted to MDH to apply for asbestos worker, asbestos site supervisor, asbestos inspector, asbestos project designer or asbestos management planner certifications.

This is a checklist to help contractors comply with the Asbestos Abatement Act and Rules (Minnesota Statutes chapter 326 and Minnesota Rules, chapter 4620).
Licensed asbestos contractors conducting regulated asbestos-related work must complete and submit the following form to MDH five (5) calendar days before the project begins.
This form is also used for changing any information from the original notification. Check "Amended" at the top of the form, change the necessary information, and then circle or highlight the new information so MDH can readily identify it.
Asbestos contractors performing only air monitoring for a project must complete and submit the following form to MDH.
Training course providers must complete and submit the following form to apply for a training course permit.
Training course providers must notify MDH 14 calendar days before the course begins with the following form.
Training course providers must provide MDH a list of students within 48 hours of the completion of the course with this form.
