Who needs to take the Lead Independent Examination?
The following disciplines require succesful completion of the Lead Independent Examination:
- Lead Supervisor
- Lead Inspector
- Lead Risk Assessor
You must achieve a score of 70% or better to successfully complete the exam.
How do I take the Lead Independent Examination?
To take the Lead Independent Examination, fill out and submit the Lead Independent Exam Registration Form (PDF: 34KB/ 1 page) to MDH.
Where do I take the Lead Independent Examination?
The Lead Independent Examination is offered at MDH's Snelling Office Park, 1645 Energy Park Drive, St. Paul, Minnesota, 55108, on the following dates in 2012:
- January 18
- February 15
- March 21
- April 18
- May 16
- June 20
- July 18
- August 15
- September 19
- October 17
- November 21
- December 19
Testing for all dates begins at 9:00 AM and ends at 11:00 AM.
What do I need to take the Lead Independent Examination?
Bring a copy of your training course diploma and a picture ID. There is no fee for the examination and parking is free at Snelling Office Park. Be sure to check the computer kiosk by the entry to find the room the test is being offered in.
What if I fail the Lead Independent Examination?
If you fail the examination, you may re-take it after 24 hours have passed. Contact MDH to set up an appointment to re-take the examination.
