Lead Independent Examination

Who needs to take the Lead Independent Examination?
The following disciplines require succesful completion of the Lead Independent Examination:
- Lead Supervisor
- Lead Inspector
- Lead Risk Assessor
You must achieve a score of 70% or better to successfully complete the exam.
How do I take the Lead Independent Examination?
To take the Lead Independent Examination, fill out and submit the Lead Independent Exam Registration Form (PDF: 34KB/ 1 page) to MDH.
Where do I take the Lead Independent Examination?
NEW In 2013, the Lead Independent Examination will be offered at the main MDH offices in the Orville Freeman Building, 625 Robert Street N, St. Paul, MN 55155. Metered parking is available on the Orange Level (top level) of the Centennial Parking Ramp (behind MDH) or across the street at Lots U and W behind the Harold Stassen Building (600 Robert Street North).

All testing sessions begin at 9:00 AM and end at 11:00 AM.
What do I need to take the Lead Independent Examination?
Bring a copy of your training course diploma and a picture ID. There is no fee for the examination.
Parking is metered at the Centennial Parking Ramp and Lots U and W behind the Harold Stassen Building. Payment options include Visa/MasterCard credit and debit cards, $1 and $5 dollar bills, as well as coins at a rate of $1.50 per hour or $6.00 for an all-day rate.
What if I fail the Lead Independent Examination?
If you fail the examination, you may re-take it after 24 hours have passed. Contact MDH to set up an appointment to re-take the examination.
