• Some information, such as property identification number or lot, block, and addition name, may not have been provided on the certificate.
• Some information, such as property address, may have changed since the certificate was filed.

If the number or status of any wells on the property have changed since the most recently filed well disclosure certificate, a new well disclosure certificate must be filed at the time of the next property transfer.
