A. OUTREACH

Outreach Activities include one-to-one and group activities designed to inform members of the target population about family planning and all family planning services available in the geographic area. Group outreach activities include presentations and classes at places such as group homes or colleges that provide factual information on reproductive and sexual health, including pregnancy prevention and all contraceptive methods. They may also include information and exercises designed to enhance the individual's decision-making, communication skills, and healthy lifestyles. Include persons for whom no client chart was opened.

B. COUNSELING

Family Planning Counseling means the provision on an individual basis of factual information on pregnancy planning and all contraceptive methods, and the use of non-directive interview techniques to promote optimum health relative to pregnancy planning and contraception.

C. FAMILY PLANNING METHOD SERVICES

Family Planning Method Services means the provision to a service recipient of her/his family planning method of choice, following counseling.

1. Unduplicated Number of Females Receiving Medical Family Planning Methods Services by Age Group, Race, and Ethnicity, and for whom a client chart** exists.

Medical Family Planning Method Services means physical examination by a physician/nurse practitioner/certified nurse midwife and lab services provided prior to the provision of a family planning method (regardless of the method chosen). These services may be provided in public clinic settings or in private offices, through reimbursement contracts.
