Minnesota Children with Special Health Needs (MCSHN)

CLINIC PROGRAM

Outreach clinics are a traditional component of the MCSHN (pronounced "mission") program. These clinics provide quality medical and rehabilitation assessments for children with suspected or diagnosed special health needs. The clinics serve to complement area health care and are located in communities where these services are not available. MCSHN clinics are staffed by a multidisciplinary team of specialists with pediatric expertise.

MCSHN clinic coordinators and district staff are available to explore the development of new clinics or alter existing clinics. New clinics will be identified by a review of available community resources and by community needs assessments.

WHO MAY ATTEND CLINICS

Children or youth up to age 21 who reside in Minnesota may attend a clinic. Children under age 18 must be accompanied by at least one parent or legal guardian. A consent for the clinic assessment will be obtained for each child seen. The consent must be signed by the parent, legal guardian or adult patient. Teachers, therapists, friends or others may also attend clinic, if desired.

Appointments are on weekdays during usual business hours. Appointments are required and whenever possible, are scheduled at a time that best fits personal needs. Notify clinic staff if you have any special time restrictions.

REFERRALS TO CLINIC

Anyone can make a referral to clinic. This includes parents, doctors, rehabilitation personnel, school teachers and nurses, Head Start staff, social service personnel, Health Maintenance Organizations (HMO's) and others. For an appointment, contact the individual listed in the Directory of Clinic Service.

Families are encouraged to obtain the appropriate insurance or managed care referral before coming to the Habilitation clinics.

CHOICE OF CARE

Parents or caretakers may obtain care for further evaluation or treatment as they desire and in accordance with their insurance or third party plan requirements. There is no obligation to seek recommended follow-up care from providers they have seen at clinic. They may seek the follow-up care from their regular primary care doctor, a local specialist or MCSHN clinic specialists.

OTHER CLINIC INFORMATION

Some clinics require children and parents to be at the clinic much of the day. In other clinics, children and parents are in and out in about an hour or so. Check each clinic's description for details. We encourage parents to bring toys, books or other activities to entertain their children while waiting.

We recommend that comfortable clothes are worn. Some clinics involve modified physical examinations. It is acceptable to bring a robe or other garment for the examination.

Lunches are not provided. Snacks may be available at some clinics. If both morning and afternoon attendance is required, we will advise you of nearby eating facilities.

We encourage parents to provide copies of important records that may be of help to the clinic team members. These records could be medical reports, therapy plans, school Individual Education Plans (IEPs), x-rays or other similar documents.

Occasionally, a clinic may be canceled at the last moment. This is almost always a weather related situation. We will make every effort to call families if a clinic is canceled. If you suspect a clinic may be canceled and have not been contacted, call the appointment individual, the MCSHN district consultant for your area or MCSHN at 1-800-728-5420.

CLINIC REPORTS

A typed report of all clinic consultant findings and recommendations is prepared and sent to providers or professionals as approved by parents, guardians or the adult patient. Parents can also receive a copy of the clinic report if they request one.

INFORMATION AND ASSISTANCE LINE

The Information and Assistance Line is for families, health care providers, public health nurses, teachers, social workers and anyone who needs help identifying and locating resources for children with special health needs. It offers information on services and resources provided by public and private agencies. Both national and state information is available.
