Help Me Grow- Infant and Toddler Intervention Services
Minnesota's early intervention system - Help Me Grow - includes two programs for eligible children. Infant and Toddler Intervention are special services and supports for children birth through age two and their families. Preschool Special Education is for eligible children ages three to five years. Eligible Minnesota children can receive services in their home, child care setting or school. Services are free to eligible families regardless of income or immigrant status.

If you have concerns about your child's development, talk to your health care provider, call 1-866-693-GROW (4769) or use the online referral form to refer your child to Help Me Grow for more assistance.

Follow-Along Program
Babies and young children grow and learn at their own rate. Some children, though, need special help to grow up healthy and learn skills such as sitting, walking, or talking. The Follow Along Program can help you know if this is true for your child.

For the referral of a child, contact your local public health agency. For information on the Follow Along Program contact the Follow Along Program Coordinator at 651-201-3641.

Medical Home
A Medical Home is where primary doctors and families work together to improve health outcomes and quality of life for children and youth with special health care needs and their families. The Minnesota Medical Home Learning Collaborative is a quality improvement project focused on primary care, coordinated by the Children and Youth with Special Health Needs Program in collaboration with several other state programs and organizations. For information on Medical Home contact Children and Youth with Special Health Needs at 651-201-3650 or 1-800-728-5420.
