Instructions

1. Select your Agency from the drop-down list.

If you are completing a multi-county plan for a Community Health Service (CHS), select just one of the counties from the list. Upon completion of the Nutrition Education Plan, you will be asked to list all of the contributing agencies.

2. Select your Nutrition Consultant.

If you are unsure of your agency's Nutrition Consultant, a link to a reference listing of Nutrition Consultants and their Local Agencies is provided.

3. Fill in your Contact Information, including Name and E-Mail Address.

4. Click the Contact Nutrition Consultant button to submit your request.

You should receive your username and password within two business days. If you don't, it may be due to your Nutrition Consultant being out of the office. After two business days, please select another Nutrition Consultant from the list and resubmit your request.
