Benefits Issued & Certs Performed b/n 7 PM and 7 AM (PDF)
The Infoview report template this document describes has five tabs and provides information about benefits being issued and certs being performed between the non-standard WIC hours of 7:00 PM and 7:00 AM at the specified agencies and during the specified time period.

The Infoview report template this document describes has five tabs and provides information about benefits being issued and certs being performed at the specified agencies and during the specified time period. This may also be helpful in auditing WIC services being performed on weekends or non-WIC days.

Benefits Issued on Date at Clinic (PDF)
The Infoview report template this document describes provides a list of all benefits issued on a specified date at a specified clinic. It includes a list and count of participants by State WIC ID with WIC Type, issuance date and time, benefits numbers, staff who issued the benefits and the void date (if applicable).

Benefits Redeemed in Following Fiscal Year (PDF)
The Infoview report template this document describes has one tab and was specifically developed to count the number of benefits issued for a federal fiscal year (FFY) and redeemed the following FFY. However, it could be used to ascertain the number of benefits issued during any time period and redeemed after a specified date.

Cash-Value Benefits Redemption Activity (PDF)
The Infoview report template this document describes has one tab. Up through August 2014 this report was saved to the FileZilla (FTP) site and was named Fruit & Vegetables Redemption Activity. It provides both State and Local Agency information about fruit and vegetable cash-value benefits (CVBs). The output provides a count of issued, redeemed, and unused CVBs, along with the percentage redeemed, for women, children and infants for the specified participation month. (Revised 4.11.16)

Certified Infants with Subsequent Issuance at Other Agency(PDF)
The Infoview report template this document describes has two tabs. The first tab is a list and count infants and postpartum women who were certified at the specified agency during the specified time period whose subsequent set(s) of benefits were issued at another agency. The second tab is a list of all infants certified for the first time at the specified agency during the specified time period.

Count of Participants & Households Issued Benefits by Date & Time(PDF)
The Infoview report template this document describes has two tabs and provides a count of participants and households who were issued benefits during the specified time period by date and hour of the day. Tab 1 provides a count based on all benefits issued. Tab 2 provides a count based only on nutrition visits (benefit pick-ups); participants who were issued benefits at a certification or Mid-certification Assessment (MCA) on the same date as the benefits were issued aren’t included in the count. The report outputs are grouped by Clinic ID. (previously BENEFIT ISSUANCE BY DATE & TIME - revised 4.11.16)

Count of Participants & Households Issued Benefits by Week, Day & Date(PDF)
The Infoview report template this document describes has two tabs and provides a count of participants and households who were issued benefits during the specified time period based on the week in the year (see NOTES), day and date. Tab 1 provides a count based on all benefits issued. Tab 2 provides a count based only on nutrition visits (benefit pick-ups); participants who were issued benefits at a certification or Mid-certification Assessment (MCA) on the same date as the benefits were issued aren’t included in the count. The report outputs are grouped by Clinic ID. (4.7.16)

Currently Certified by City of Residence (PDF)
The Infoview report template this document describes has eight tabs and provides a count of currently certified participants and households by their current city of residence. The first four tabs provide counts for all cities in which participants belonging to the specified agencies currently live. The second four tabs provide counts for specified cities (this is OPTIONAL – the report template can be run without specifying individual cities – see NOTES section). These reports are also based on Race/Ethnicity, WIC Type, and the Clinic where the last set of benefits was issued. All report outputs are grouped by Agency ID.

Identity & Residency Proof: Pending Proof or Other-Write Note (PDF)
The Infoview report template this document describes has two tabs and provides a list of participants, certified during a specified time period, with either PENDING PROOF or OTHER-WRITE NOTE selected for their ID Proof or Residency Proof. These participants were issued more than the one month of benefits allowed by Federal Regulations without proof of identification or proof of residence. The report identifies both the staff who certified the participant and the staff who issued the subsequent set(s) of benefits. It is grouped by clinic ID and State WIC ID.

Infants & Children with Future Appointments Who Have Not Picked Up (PDF)
The Infoview report template this document describes provides a list of currently certified infants and children who have an appointment scheduled during a specified future time period and have missed nutrition visits/benefit pickups based on their last set’s Last Date to Use (LDTU) being before the first date of the current month.

Infants by Age Issued Benefits for Specified Participation Month (PDF)
The Infoview report template this document describes provides has two tabs and was created due to a request from the Farmer’s Market organization. The first tab provides a list and total count of infants, and their age in months at the time benefits were issued, for a specified participation month. The second tab is a count by age in months.

Last Set Issued at Clinic Labels (PDF)
The Infoview report template this document describes has five tabs. The first tab provides a count and list of currently certified participants who were issued their last set of benefits at a specified clinic. The next four tabs can be used to create labels: household labels addressed to “RESIDENT”, household labels addressed to the Authorized Representative, participant labels addressed to the participant, and participant labels addressed to the Authorized Representative. All Labels tabs include mailing address, city, state and zip code.

Last Set Issued at Clinic by City Labels (PDF)
The Infoview report template this document describes has five tabs. The first tab provides a count and list of currently certified were issued their last set of benefits at a specified clinic and whose mailing address includes the city specified by the report user. The next four tabs can be used to create labels: household labels addressed to “RESIDENT”, household labels addressed to the Authorized Representative, participant labels addressed to the participant, and participant labels addressed to the Authorized Representative. All Labels tabs include mailing address, city, state and zip code.

Lost & Stolen (PDF)
The Infoview report template this document describes has two tabs and provides a list of participants who had benefits marked as lost or stolen during a specified time period. The report also provides the Staff ID of the person who marked them lost/stolen and redemption information if applicable. The second tab provides a count of redeemed benefits marked lost/stolen and a count of participants by Staff ID.

Mailed Benefits (PDF)
The Infoview report template this document describes provides a list of participants who were mailed benefits issued during a specified time period by the specified agencies. The report output provides both the date and time the mailed benefits were issued along with the Staff ID of the person who issued them.

Mailed Benefits: Counted, Replaced & Redeemed (PDF)
The Infoview report template this document describes has five tabs. The first tab provides a list and count of participants to whom benefits were mailed during the specified participation month by the specified agency. The Tab 2 provides a list and count of these participants whose mailed benefits were voided and whether they were counted as of the current date (see Notes). Tab 3 provides a subset of the participants whose mailed benefits were voided and who subsequently had a replacement set issued for that same month. Tab 4 provides redemption information for each mailed benefit for participants listed in Tab 1. The last tab provides total redemption for each participant listed in Tabs 1 and 4.

Mailed Two or More Consecutive Sets of Benefits (PDF)
The Infoview report template this document describes has three tabs. The first tab lists participants who were mailed two or more consecutive sets of benefits during the specified time period by the specified agency. The second tab is an Excel format-friendly version of the information provided in Tab 1. The last tab is a count of the number of sets of benefits mailed.

Missed Nutrition Visit/Benefit Pickup (PDF)
The Infoview report template this document describes has three tabs and provides a list and count of participants who are eligible to receive benefits and have missed a nutrition visit/benefit pick-up during a specified time period. It also provides lists that can be used to create labels based on household with the authorized representative’s name or by participant with the participant’s name. The report is grouped by the clinic where the participant is currently assigned.

The Infoview report template this document describes has three tabs and provides a list and count of participants who are eligible to receive benefits and have missed a nutrition visit/benefit pick-up during a specified time period. It also provides lists that can be used to create labels based on household with the authorized representative’s name or by participant with the participant’s name. The report is grouped by the clinic where benefits were last issued to the participant.

More than One Set of Benefits Issued for Participation Month (PDF)
The Infoview report template this document describes has four tabs. Tabs 1 and 3 provide a list and count of infants and all other WIC types, respectively, who received more than one set of benefits for a specified participation month (excluding those that received two sets of benefits due to syncing packages). Tabs 2 and 4 provide detailed information for each benefit that is included in the multiple sets of benefits issued to those participants.

One Call: Missed Nutrition Visit (Pick-up) (PDF)
The Infoview report template this document describes is for agencies participating in the One Call automated messaging project and utilizing the Local Use Question in Position 6. This report has two tabs. The first tab is a reference report that can be used to obtain the value IDs necessary to identify participants who are participating in the One Call automated messaging project. The second tab provides a list of participants who have missed a nutrition visit (or benefit pick-up) during a specified time period who want to be notified by One Call for missed nutrition visit/benefit pick-up by voice messages, texts, or emails.

Participants Issued Benefits by Staff ID (PDF)
The Infoview report template this document describes provides a list and count of participants based on specified WIC Types who were issued benefits during a specified time period at a specified agency (or agencies). (Revised 2.1.16)

Redeemed CVBs (PDF)
The Infoview report template this document describes has five tabs. The first two tabs provide a list and count of participants who were issued cash-value benefits (CVBs) based on WIC Type for a specified participation month and specified agencies. Tab 3 provides a count of the CVBs issued by Agency ID and WIC Type while Tab 4 provides a count of the participants issued CVBs. The last tab provides the total redemption amount for redeemed CVBs.

Statewide Count & Percentage of Eligible Infants Not Receiving CVBs by Agency (PDF)
The Infoview report template this document describes has one tab and is a snapshot of infants who are 9 to 12 months old as of the current date and have received a set of benefits with a Printed First Date to Use (PFDTU) between 9 and 10 months of age. The output provides a statewide count of infants eligible to receive fruit and vegetable cash-value benefits (CVBs) by agency; a count of these same infants who were NOT issued CVBs; and the percentage of eligible infants who did NOT receive CVBs.
