MN WIC Program

These Guidance Documents provide detailed information about Infoview Report Templates created by the State Office for use by Infoview users. They explain how the report was created and may help to answer questions about how the report works.

Infoview users are strongly encouraged to review the report's Guidance Document (up to the TECHNICAL NOTES section) before running the Report Template in Infoview.



  • Benefit Issuance by Date & Time (PDF: 345KB/16 pages)
    The Infoview report template this document describes has six tabs and provides a list and count of participants and households issued benefits during a specified time period, and at specified clinics, based on the hour of the day. The first three tabs include all participants issued benefits, regardless of whether the benefits were issued at a Nutrition Visit (NV) or at the end of a Certification or Mid-certification Assessment (MCA) appointment. The last three tabs include only those participants who were issued benefits and did not have an appointment during that time period.
  • Benefits Issued on Date at Clinic (PDF: 191KB/4 pages)
    The Infoview report template this document describes provides a list of all benefits issued on a specified date at a specified clinic. It includes a list and count of participants by State WIC ID with WIC Type, issuance date and time, benefits numbers, staff who issued the benefits and the void date (if applicable).
  • BF Amount & Formula Issued on Last Set of Benefits (PDF: 285KB/11 pages)
    The Infoview report template this document describes has three tabs. The first tab provides a list and count of infants along with their Breastfeeding (BF) Amount and the amount of formula issued on their last set of benefits. Tab 2 provides a count by clinic and Tab 3 provides a count by Race/Ethnicity.
  • Cash-Value Benefits Redemption Activity (PDF: 185KB/5 pages)
    The Infoview report template this document describes has one tab. Up through August 2014 this report was saved to the FileZilla (FTP) site and was named Fruit & Vegetables Redemption Activity. It provides both State and Local Agency issuance and redemption information for the fruit and vegetable cash-value benefits (CVBs) based on the specified participation month.
  • Children Issued $6.00 CVBs (PDF: 293KB/13 pages)
    The Infoview report template this document describes has four tabs. The first and second tabs are based on $6.00 cash value benefits (CVBs) issued to currently certified children who belong to a specified agency and whose last set of benefits were issued on or after May 5, 2014. Tab 1 provides a list and count of these children. Tab 2 provides a count by clinic with the percentage of children issued the $6.00 CVB compared to the total number of children issued benefits. The third and fourth tabs provide the same information as Tabs 1 and 2 except that the information applies to all agencies statewide and are based on benefits issued on or after a date specified by the report user. The counts and percentage in Tabs 3 and 4 are based on agency instead of clinic.
  • Currently Certified by City of Residence (PDF: 304KB/14 pages)
    The Infoview report template this document describes has eight tabs and provides a count of currently certified participants and households by their current city of residence. The first four tabs provide counts for all cities in which participants belonging to the specified agencies currently live. The second four tabs provide counts for specified cities (this is OPTIONAL – the report template can be run without specifying individual cities – see NOTES section). These reports are also based on Race/Ethnicity, WIC Type, and the Clinic where the last set of benefits was issued. All report outputs are grouped by Agency ID.
  • Eligible Infants NOT Issued CVBs on Last Set of Benefits (PDF: 307KB/15 pages)
    The Infoview report template this document describes has four tabs. The first and second tabs are based on a specified agency and an issuance date on or after July 14, 2014. Tab 1 provides a list and count of currently certified infants who were eligible to receive Cash-Value Benefits (CVBs) on their last set of benefits. Tab 2 provides a count by clinic with the percentage of eligible infants not issued CVBs on their last set of benefits compared to the total number of eligible infants issued benefits. The third and fourth tabs provide the same information as Tabs 1 and 2 except that the information applies to all agencies statewide and are based on benefits issued on or after a date specified by the report user. The counts and percentage in Tabs 3 and 4 are based on agency instead of clinic.
  • Food & Formula Item(s) Issued (PDF: 302KB/14 pages)
    The Infoview report template this document describes has six tabs. The first tab is a reference report that can be used to obtain the Food Item ID(s) necessary to identify the participants receiving certain food or formula items. It also indicates whether the item is currently available for issuance (Active), when it became active (Effective Date), and when and if it became inactive (Expiration Date). Tabs 2–4 provide lists and counts of infants, children and women who have received the specified food or formula item(s) on a set of benefits with a Printed First Date to Use (PFDTU) during the specified time period. Tab 5 can be used to create labels for participants listed in Tabs 2–4. Tab 6 provides a cross-tab count by Food Item ID and WIC Type. Tabs 2-4 and 6 are grouped by Agency ID.
  • Food/Formula Issued on Last Set of Benefits (PDF: 310KB/16 pages)
    The Infoview report template this document describes has eight tabs. The first two tabs are reference reports that can be used to obtain the Food Item ID(s) necessary to identify the participants issued a certain food item and/or non-medical formula. Tabs 3 – 5 provide a list and count of currently certified participants (infants, children, and women) who received the food item(s) and/or non-medical formula(s) on their most recent set of benefits; grouped by Agency ID and Clinic ID. Tabs 6 – 8 provide a count of infants, children, and women issued the food item(s) and/or non-medical formula(s) on their last set of benefits by food item/non-medical formula and clinic site.
  • Identity & Residency Proof: Pending Proof or Other-Write Note (PDF: 193KB/8 pages)
    The Infoview report template this document describes has two tabs and provides a list of participants, certified during a specified time period, with either PENDING PROOF or OTHER-WRITE NOTE selected for their ID Proof or Residency Proof. These participants were issued more than the one month of benefits allowed by Federal Regulations without proof of identification or proof of residence. The report identifies both the staff who certified the participant and the staff who issued the subsequent set(s) of benefits. It is grouped by clinic ID and State WIC ID.
  • Last Set Issued at Clinic Labels (PDF: 214KB/6 pages)
    The Infoview report template this document describes has five tabs. The first tab provides a count and list of currently certified participants who were issued their last set of benefits at a specified clinic. The next four tabs can be used to create labels: household labels addressed to “RESIDENT”, household labels addressed to the Authorized Representative, participant labels addressed to the participant, and participant labels addressed to the Authorized Representative. All Labels tabs include mailing address, city, state and zip code.
  • Last Set Issued at Clinic by City Labels (PDF: 257KB/7 pages)
    The Infoview report template this document describes has five tabs. The first tab provides a count and list of currently certified were issued their last set of benefits at a specified clinic and whose mailing address includes the city specified by the report user. The next four tabs can be used to create labels: household labels addressed to “RESIDENT”, household labels addressed to the Authorized Representative, participant labels addressed to the participant, and participant labels addressed to the Authorized Representative. All Labels tabs include mailing address, city, state and zip code.
  • Lost & Stolen (PDF: 55KB/3 pages)
    The Infoview report template this document describes provides a list of participants who had lost or stolen benefits during a specified time period and the Staff ID of the person who marked them lost/stolen. Also provides a count of participants with lost or stolen benefits by Staff ID.

    The Infoview report template this document describes has four tabs. Tabs 1 – 3 provide a list and count of participants (infants, children, and women) who received medical formula on a set of benefits during a specified time period. Tab 4 provides a list of the infants, children, and women in Tabs 1 – 3.

The first tab is a reference report that can be used to obtain the Food Item ID(s) necessary to identify the participants issued a certain medical formula. Tabs 2 – 4 provide a list and count of currently certified participants (infants, children, and women) who received medical formula on their most recent set of benefits; grouped by Agency ID and Clinic ID. Tabs 5 – 7 provide a count of infants, children, and women issued medical formula on their last set of benefits by type of medical formula and clinic site.
  • Missed Nutrition Visit/Benefit Pickup (PDF: 294KB/14 pages)
    The Infoview report template this document describes has three tabs and provides a list and count of participants who are eligible to receive benefits and have missed a nutrition visit/benefit pick-up during a specified time period. It also provides lists that can be used to create labels based on household with the authorized representative’s name or by participant with the participant’s name. The report is grouped by the clinic where the participant is currently assigned.
  • Missed Nutrition Visit/Benefit Pickup by Last Clinic Issued (PDF: 321KB/15 pages)
    The Infoview report template this document describes has three tabs and provides a list and count of participants who are eligible to receive benefits and have missed a nutrition visit/benefit pick-up during a specified time period. It also provides lists that can be used to create labels based on household with the authorized representative’s name or by participant with the participant’s name. The report is grouped by the clinic where benefits were last issued to the participant.
  • More than One Set of Benefits Issued for Participation Month (PDF: 304KB/12 pages)
    The Infoview report template this document describes has four tabs. Tabs 1 and 3 provide a list and count of infants and all other WIC types, respectively, who received more than one set of benefits for a specified participation month (excluding those that received two sets of benefits due to syncing packages). Tabs 2 and 4 provide detailed information for each benefit that is included in the multiple sets of benefits issued to those participants.
  • One Call: Missed Nutrition Visit (Pick-up) (PDF: 268KB/11 pages)
    The Infoview report template this document describes is for agencies participating in the One Call automated messaging project and utilizing the Local Use Question in Position 6. This report has two tabs. The first tab is a reference report that can be used to obtain the value IDs necessary to identify participants who are participating in the One Call automated messaging project. The second tab provides a list of participants who have missed a nutrition visit (or benefit pick-up) during a specified time period who want to be notified by One Call for missed nutrition visit/benefit pick-up by voice messages, texts, or emails.

    The Infoview report template this document describes has five tabs. The first two tabs provide a list and count of participants who were issued cash-value benefits (CVBs) based on WIC Type for a specified participation month and specified agencies. Tab 3 provides a count of the CVBs issued by Agency ID and WIC Type while Tab 4 provides a count of the participants issued CVBs. The last tab provides the total redemption amount for redeemed CVBs.
