BF Peer Referrals (PDF)
The Infoview report template this document describes has two tabs. The first tab is a reference report that can be used to obtain the Organization/Referral ID(s) necessary to run the report in Tab 2. The second tab provides a list of participants who received a peer referral contact during a specified time period.

No Peer Assigned Referrals (NoPAR) (PDF)
The Infoview report template this document describes has four tabs. The first tab is a reference report that can be used to obtain the Organization ID(s) necessary to identify the agency referrals to include in the report. The second tab is a list of clients referred during the specified time period for the specified agency. The third tab is a list of those from Tab 2 who have been assigned a peer. The fourth tab lists those from Tab 2 who have not been assigned a peer. The main purpose of this report is to make sure that all clients referred are assigned a peer.

Peer Active Client List (PDF)
The Infoview report template this document describes has five tabs. The first tab is a reference report that can be used to obtain the BF Counselor ID(s) necessary to identify the peer counselors for whom the report should be run. The second tab is a list of active clients for all specified peer counselor(s). The third through fifth tabs are a report listing active peers by each peer counselor specified, sorted in three ways.

Peer Contact Type and Frequency Report (PDF)
The Infoview report template this document describes has four tabs. The first tab is a reference report that can be used to obtain the BF Counselor ID(s) necessary to identify the peer counselors for whom the report should be run. The second tab is a count of entries and estimated contacts by contact type. The third and fourth tabs are a report tabulating contacts by type (face to face, telephone and text/email) and timing of contact (prenatal, postpartum 0 – 3 months and postpartum 4 – 12 months), including percentages for each category. Tab three contains the summary counts and tab four the counts broken out by peer counselor.

Peer Active and Closed Client List (PDF)
The Infoview report template this document describes has two tabs. The first tab is a reference report that can be used to obtain the BF Counselor ID(s) necessary to identify the peer counselors for whom the report should be run. The second tab is a list of clients for the specified peer counselor(s) and all peer contacts, which identify the beginning or end of peer counseling, since a specified date.

Peer Contact Summary Form (for infants at 3 months and 12 months) (PDF)
The Infoview report template this document describes has two tabs. The first tab is a reference report that can be used to obtain the BF Peer Counselor ID(s) necessary to identify the peer counselors to include in the report. The second tab is a list of these peer counselors’ clients whose infants were born during a specified time period. The main purpose of this report is to create a list to be used by peer counselors to report their contacts with clients when babies are 3 or 12 months or whenever clients are completed (at closure)..

Spot Check Report (PDF)
The Infoview report template this document describes has four tabs. The first tab is a reference report that can be used to obtain the BF Counselor ID(s) necessary to identify the peer counselors for whom the report should be run. The second tab lists clients with completed spot checks in the specified time period. The third and fourth tabs list active and recently completed clients who have not had a spot check during the specified time period.
