MN WIC Program

These Guidance Documents provide detailed information about Infoview Report Templates created by the State Office for use by Infoview users. They explain how the report was created and may help to answer questions about how the report works.

Infoview users are strongly encouraged to review the report's Guidance Document (up to the TECHNICAL NOTES section) before running the Report Template in Infoview.



  • 3 or 6 Month Infant Contacts (PDF: 204KB/6 pages)
    The Infoview report template this document describes has two tabs and can be used to help identify participants for 3-month and 6-month infant Nutrition Education contact audits. It provides a list and count of currently certified infants whose age is currently between 4-5 months and 7-8 months old. In order to help ensure the participants listed should have had a 3 or 6 month infant contact, their last set's Last Date to Use (LDTU) must be greater than the current date and they must have been certified within 45 days of their birthdate. Both tabs are grouped by Clinic ID.

  • Appointments with Interpreter Needed (PDF: 176KB/5 pages)
    The Infoview report template this document describes has two tabs. It provides a list and count of participants, with an appointment scheduled during a specified time period, who have the Need Interpreter checkbox selected in Demographics. The second tab provides a count of participants by language and clinic ID.
  • Benefit Issuance by Date & Time (PDF: 345KB/16 pages)
    The Infoview report template this document describes has six tabs and provides a list and count of participants and households issued benefits during a specified time period, and at specified clinics, based on the hour of the day. The first three tabs include all participants issued benefits, regardless of whether the benefits were issued at a Nutrition Visit (NV) or at the end of a Certification or Mid-certification Assessment (MCA) appointment. The last three tabs include only those participants who were issued benefits and did not have an appointment during that time period.
  • Certifications Completed by One Staff Person (PDF: 274KB/11 pages)
    The Infoview report template this document describes has two tabs and was developed to potentially assist agencies with assessing whether Separation of Duties requirements are being met. The first tab provides a list and count of participants for whom one staff person is recorded for all required steps of a certification performed during a specified time period. The second tab lists other household members, of those participants listed in Tab 1, who were certified during the same time period and whose certification does not have a staff person recorded for income. These are participants that most likely also had only one staff person perform their certification.
  • Certified Participants (PDF: 244KB/9 pages)
    The Infoview report template this document describes has four tabs. The first three tabs provide lists and counts of all certifications, recertifications, and new certifications performed during a specified time period. Tab 4 provides a count of certifications by Clinic ID and WIC Type.
  • Currently Certified by City of Residence (PDF: 304KB/14 pages)
    The Infoview report template this document describes has eight tabs and provides a count of currently certified participants and households by their current city of residence. The first four tabs provide counts for all cities in which participants belonging to the specified agencies currently live. The second four tabs provide counts for specified cities (this is OPTIONAL – the report template can be run without specifying individual cities – see NOTES section). These reports are also based on Race/Ethnicity, WIC Type, and the Clinic where the last set of benefits was issued. All report outputs are grouped by Agency ID.
  • First Time Pregnant Women 19 & Older (PDF: 217KB/7 pages)
    The Infoview report template this document describes has three tabs and provides a list and count of women who were certified for the WIC Program during a specified time period and are new to WIC, pregnant for the first time, and 19 years or older. The third tab provides a reference for Race/Ethnicity codes and their descriptions.
  • Infants & Children by Age (PDF: 290KB/12 pages)
    The Infoview report template this document describes has five tabs and provides a total count and list of currently certified infants and children 0 – 11, 12 – 23, 24 – 35, 36 – 47, and 48 – 60 months old as of the current date.
  • Insurance Type (PDF: 278KB/12 pages)
    The Infoview report template this document describes has three tabs and is based on completion of the Insurance Type field in the Demographics AdditionalInfo1 tab. It provides a count by WIC Type and type of insurance, along with a percentage of the total count by type of insurance, for all currently certified (or enrolled) participants and for all participants certified during a specified time period. The third tab provides a list of participants certified during the specified time period and includes the Staff ID of the certifier.
  • LDTU on 28th or 29th (PDF: 185KB/5 pages)
    The Infoview report template this document describes has two tabs and provides a count and list of currently certified participants whose last set’s Last Date to Use (LDTU) is on the 28th or the 29th by specified agency and statewide.
  • ME Chart Audit (PDF: 358KB/9 pages)
    The Infoview report template this document describes has three tabs. The first tab provides a list and count of participants issued benefits during a specified time period and for a specified participation month. The information provided includes a high-level overview of that issuance and the participant’s most recent certification. Tabs 2 and 3 provide information about the income record associated with the participant’s most recent certification when obtainable from the income tables.
  • Moms & Infants (PDF: 150KB/4 pages)
    The Infoview report template this document describes provides a list and count of women with infants born during a specified time. If a two-way link exists between the mom’s and infant’s records both mom and infant display. Otherwise, only the woman displays.
  • One Call: Future Appointments (PDF: 221KB/6 pages)
    The Infoview report template this document describes is for agencies participating in the One Call automated messaging project and utilizing the Local Use Question in Position 6. This report has two tabs. The first tab is a reference report that can be used to obtain the value IDs necessary to identify participants who are participating in the One Call automated messaging project. It also has a list of Appointment Type IDs and their descriptions, which can be used to help agencies and One Call specify the type of appointments that require reminders. The second tab provides a list of participants with appointments scheduled during a specified future time period who want to be notified by One Call for appointment reminder voice messages, texts, or emails.

    The Infoview report template this document describes is for agencies participating in the One Call automated messaging project and utilizing the Local Use Question in Position 6. This report has two tabs. The first tab is a reference report that can be used to obtain the value IDs necessary to identify participants who are participating in the One Call automated messaging project. It also has a list of Appointment Type IDs and their descriptions, which can be used to help agencies and One Call specify the type of appointments that require contact. The second tab provides a list of participants who missed their scheduled appointment during a specified time period in the past who want to be notified by One Call for missed appointments via voice messages, texts, or emails.

    The Infoview report template this document describes is for agencies participating in the One Call automated messaging project and utilizing the Local Use Question in Position 6. This report has two tabs. The first tab is a reference report that can be used to obtain the value IDs necessary to identify participants who are participating in the One Call automated messaging project. The second tab provides a list of participants who have missed a nutrition visit (or benefit pick-up) during a specified time period who want to be notified by One Call for missed nutrition visit/benefit pick-up by voice messages, texts, or emails.

The first tab provides a list and count of pregnant women with an Expected Delivery Date during a specified time period, including whether they are expecting multiples. The second tab provides a count of pregnant women by Clinic ID.
  • Pregnant Women by Age (PDF: 205KB/6 pages)
    The Infoview report template this document describes has two tabs. The first tab lists all currently certified pregnant women sorted by age while the second tab counts all participants by age and age groups. Both are grouped by Agency ID.
  • Pregnant Women - Third Trimester (PDF: 197KB/5 pages)
    The Infoview report template this document describes has two tabs. The first tab provides a list and count of currently certified women who are in their third trimester while the second tab can be used to create labels for these participants.
  • Presumptive Eligibility (PDF: 171KB/3 pages)
    The Infoview report template this document describes provides a list of participants for whom Presumptive Eligibility – 90 Day was selected for the income Proof during a specified time period.
  • Resource Management (PDF: 279KB/11 pages)
    The Infoview report template this document describes has six tabs and provides a count and list of participants due for Mid-Certification Assessments (MCAs), Recerts, and Delivery; and participants becoming categorically ineligible for WIC during the specified time period; grouped by Agency ID and Clinic ID. It also provides summary counts of participants by agency and clinic and by WIC type, agency and clinic.
  • Some-BF Women 8-11 Months Postpartum (PDF: 196KB/4 pages)
    The Infoview report template this document describes provides a list of Some-Breastfeeding women who are between 8 and 11 months postpartum as of the participation month specified by the report user. It may be useful to for auditing participant records to ensure the required Nutrition Education contacts are being provided to some-breastfeeding participants who are no longer receiving food benefits but are still being counted each month.
  • Terminated & Ineligible (PDF: 358KB/17 pages)
    The Infoview report template this document describes has six tabs and provides lists of participants terminated, determined ineligible, or who became categorically ineligible during the specified time period. It also provides the staff ID of the person who terminated a participant, or determined a participant ineligible, and counts of these participants by clinic, WIC type, and race ethnicity.
  • Women with Unlinked or One-Way Linked Infants (PDF: 205KB/6 pages)
    The Infoview report template this document describes provides a list of currently certified women who have infants that are either missing a link between the two records or have a one-way link between the two records. It also provides the name of the staff person who completed the certification.
