Home Care Provider Survey Results
Under the current Minnesota home care laws (effective as of January 2014) home care providers must be surveyed at least once every three years. Temporary licensees must be surveyed within the first fourteen months of the temporary license period in order to be eligible for a license.
Consumers (recipients of home care and their families) and providers can find the results of the Minnesota Department of Health’s surveys of home care providers at the links below.
IMPORTANT NOTE FROM MDH:
If you are looking for information on providers who have had recent enforcement actions that resulted in a suspension or closure of the license (including denial or revocation of a license), contact the Home Care and Assisted Living Program at 651-201-5273 or firstname.lastname@example.org to request this data.
MDH is aware that some home care provider survey results are currently not available on the MDH website. This occurred when providers with multiple licenses elected to combine licenses and serve clients under one license. When the inactive license in the MDH facility database was closed, the survey results no longer appeared on the website. MDH is working with our department’s IT staff to make home care providers’ survey results available to you. Thank you for your patience during this work.
