Registration Information:

The class is appropriate for new MDS Nurses. For the best experience, participants should have some experience completing the MDS. Space is limited to 70 registrations and each facility may send a maximum of two participants.

Participants must register by the end of the business day on Monday December 11, 2017, at the following link: MDH No Charge Registration. When registering please enter the name and contact information for the participant. The handouts for the class will be sent in an attachment to each participant’s email address one day prior to the class. Each participant must print and bring the training handouts with them to class. A registration confirmation will be sent via email. This is the only confirmation you will receive. Please keep this email as this is the only communication you will receive with the event details.

When the class is full, registration will close and you will not be able to register. If you are unable to register for the class and would like your name added to a wait list, send an email to health.mds@state.mn.us. Please include the participants name, the name of the facility, the participant’s phone number and business email address. If all the information is not provided, the participant’s name will NOT be added to the wait list.
