Class A Questions and Answers was revised April 2009. Click Here to access the revised document. Beginning March 3, 2008, Telephone Interview and Education Assessment calls will be placed by registered nurse staff members to all newly licensed Class F and Class A (licensed only) Home Care Providers as well as other providers who have not been surveyed by Case Mix Review staff. The purpose of these calls is to assure that licensed home care providers have access to information they need related to the requirements of their license, briefly review the rules that are most commonly out of compliance, open the door for further questions regarding the provider’s license, and offer education on the rules and regulations applicable to their license. A Guide to Home Care Services (Revised January 2008) A Guide to Home Care Services may be used to meet the orientation to home care requirements as outlined in MN Rule 4668.0805, Subp. 1, and is the curriculum mentioned in MN Rule 4668.0805, Subp. 3. The Minnesota Home Care Bill of Rights There are now two versions of the Minnesota Home Care Bill of Rights. One version to be given to assisted living clients called "Minnesota Home Care Bill of Rights for Assisted Living Clients", and a version for clients not receiving assisted living services called "Minnesota Home Care Bill of Rights". Class A Survey Results The Licensing Survey Form will be posted under Class A (Licensed Only) Survey results. Survey Results are posted after the form has been reviewed by a supervisor and mailed back to the licensee. The Correction Order Form is not posted. Survey results from follow-up visits will be posted after the form has been reviewed by a supervisor and mailed back to the Licensee.
