Availability of Uniform Consumer Information Guide for Assisted Living

Purpose:

The purpose of this information bulletin is to inform providers that the Uniform Consumer Information Guide for Assisted Living is now available. The guide must be provided by the assisted living providers to all current and prospective clients.

Effective Date: January 1, 2008

Assisted living establishments must provide the guide to current and prospective clients effective January 1, 2008. Assisted Living providers may begin to use the guide prior to the effective date of January 1, 2008.

Location of Guide and Instructions:

The Uniform Consumer Information Guide format and the instructions for completing the guide are located on the Minnesota Department of Health, Case Mix Review – Assisted Living website: http://www.health.state.mn.us/divs/fpc/profinfo/cms/als/alsindex.html.

Background:

Minnesota statutes 144G.03, Subdivision 2 (b) (9), requires that a housing with services establishment using the phrase “assisted living” meet the minimum requirements including: “makes available to all prospective and current assisted living clients information consistent with the uniform format and the required components adopted by the commissioner under section 144G.06. This information must be made available beginning no later that six months after the commissioner makes the uniform format and required components available to providers according to section 144G.06”.
