Video Purchase Information
The Minnesota Department of Health (MDH) with the assistance of Care Providers of Minnesota, has produced a video for nursing assistants that gives an overview of the federal survey process. The video entitled "The Federal Nursing Home Survey Process: The Role of the Nursing Assistant", is available in two formats, VHS and DVD.
Two copies of the video (1 VHS, 1 DVD) are being sent free of charge beginning the week of October 4, 2004, to Minnesota Nursing Homes, state provider organizations, Minnesota NAR training and testing sites, and Minnesota board and care home providers. Additional copies of the video may be purchased for $10.00 each. Please contact the Nursing Assistant Registry for more information, or see the attached order form for details.
VHS and DVD
The video is available in VHS and DVD format at a cost of $10.00 per copy. Please complete and copy the following order form, enclose a check or money order for the correct amount made out to the Minnesota Department of Health, and mail your order form and payment to:
