Twin Cities CDSMP Consortium Newsletter
The Self-Managing Chronic Conditions newsletter will be distributed on a quarterly basis by the Minnesota Department of Health on behalf of the Twin Cities CDSMP Consortium.
Recipients will include individuals in the consortium member organizations and CDSMP trained leaders. The newsletter will offer information about upcoming trainings and classes as well as program updates and leader support information. To subscribe to Managing Chronic Disease or to submit information for the newsletter, contact: health.aging@state.mn.us.
