SagePlus Clinic Updates
The SAGEplus program is coming to the close of the current five year funding cycle and will end on June 29, 2013. Please review the following steps your site will need to take in order to close the SAGEplus program.
May 15, 2013 – Stop enrolling women into SAGEplus. Continue to enroll women into Sage.
June 15, 2013 – all Life Style Interventions (LSI) and/or Diagnostic Referral follow-up appointments need to be completed.
June 29, 2013 – All SAGEplus forms mailed to MDH.
MDH has applied to the Centers for Disease Control (CDC) for another funding cycle.
If new funding is allocated for SAGEplus by CDC:
We will look into options to continue SAGEplus at selected sites between July 1, 2013 and December 31, 2013. Your Regional Coordinator will contact you, or call us with questions.
The NEW SAGEplus Program will launch January 1, 2014 – more information will be available in the fall of 2013.
