Eligibility Guidelines for SagePlus Services
Women who meet all the criteria are eligible:
- Age 40 through 64
- Receive breast and cervical cancer screening services through Sage at the same clinic and on the same day.
- Have no insurance or are underinsured*
- Income within our guidelines (see table below)

* Underinsured includes: Insurance that does not cover screening or insurance with unmet deductibles or copayments. Women on Medicare where there are uncovered expenses associated with the visit, Pap, or mammogram. Women that are on either MinnesotaCare or Medical Assistance are not eligible forSage (because both cover these screening services)
** Program income guidelines are based on the Federal Poverty Level. This is updated annually at the end of January. SagePlus' prior year guidelines remain in effect until then.
Women should be willing to:
- Participate in cardiovascular screening
- Learn about healthy lifestyles
- Consider making a few, small, self-selected changes toward a healthier life
- Return for follow-up and annual rescreening
