2011-2020 State Plan Development
We had a Core Planning Group, Disease Management Sub Committee, Treatment Sub Committee, and Prevention Sub Committee that met to help us develop the State Plan.
There were a total of 4 meetings scheduled for each Sub Committee and there were 5 meetings scheduled for the Core Planning Group.
Roles and Responsibilities
Core Planning Group
Provided a framework for the plan, common definitions, direction and assignments for the Planning Subcommittees, develop overarching goals and objectives, and a link to the HDSP Statewide Steering Committee.
Prevention Sub Committee
Worked to develop doable strategies and tactics to address primary causes of cardiovascular and stroke.
Acute Treatment Sub Committee
Worked to develop doable strategies and tactics to support early identification and acute care activation, delivery and coordination of care for heart attacks, strokes, pulmonary embolism, and heart failure.
Disease Management Sub Committee
Worked to develop doable strategies and tactics to provide ongoing care delivery and coordination by utilizing a caregiver approach.
