Application and Instructions for a Temporary License:

Persons Credentialed by NBCOT or Other States

Instructions:

Complete these steps to apply for a Temporary License on the basis of NBCOT or other state’s credential. You must be currently credentialed by NBCOT or another state to qualify for a Temporary License. This temporary license may be obtained and allow you to practice while you complete the application process for full licensing.

  • Print these instructions as a checklist (PDF: 1 page)

  • NOTE: Once MDH has received your completed temporary application, additional information and fees, your check or money order in deposited immediately. The application is then reviewed and processed. The processing time for a temporary license takes 3-7 business days AFTER all forms and supporting documents are received.

  • To check if your credential has been issued you can go to our website at: http://pqc.health.state.mn.us/hopVerify/loginAction.do
    Your name will appear on the website the day after your credential has been issued.

  • Print the temporary application form using this link:

  • Complete, sign and date the application.

  • Make out check or money order for $50.00 payable to: Treasurer: State of Minnesota. All fees are nonrefundable.

  • Make a copy of your NBCOT certificate (8 1/2 x 11) and copies of credentials from all other states in which you are registered, certified or licensed. Submit the copies with the temporary application.

  • Make a copy of the completed application and all supporting forms for your records.

  • Mail completed original temporary application, additional information and fees to the address on the application form. Include in this mailing, your completed application and fees for a full license.
