You are required to mail, email or fax a copy of your marriage certificate or court order, along with your Occupational Therapy(OT) /Occupational Therapy Assistant (OTA)license #, current address and telephone number. Processing time takes 15-30 business days and you will be notified in writing. You can also check if it has been updated at Health Occupations Program Credential Lookup.
