REGISTER
- Registration to participate in FluSafe is closed for the 2012-13 flu season.
After you register, you and your secondary contact will receive a confirmation email within 3 business days. The email will give you contact information for your MIIC Regional Coordinator.
- Follow-up with your MIIC Regional Coordinator.
Ask questions, confirm participation, and make sure you have proper access to MIIC.

PHASE 1 |
First time FluSafe participants:
(use these instructions to download a blank MIIC Flu Spreadsheet)
- Go to the MIIC application and log in
- Click on the “Template Download” link under the “Flu Spreadsheet” group
- Choose “Generate a blank template”
- Save this spreadsheet where you can access it throughout the flu season
- Enter the information for ALL employees* on the spreadsheet, you’ll need the following information:
- First and last name
- Birthdate
To help eliminate duplicates, consider also entering:
- Middle name
- City, state, and ZIP code of residence

Previous FluSafe participants:
(use these instructions to work from a list you uploaded last year)
- Go to the MIIC application and log in
- Click on the “Manage List” link under the “Lists” group
- Find your most up-to-date list, click the “Save As” button next to that list
- Using the box at the top, rename the list “FluSafe 2012” (add the facility type if applicable)
- You will now see the “FluSafe 2012” list with your other lists, you may open it and delete any employees* who no longer work in your facility
- Go to the menu and click on the “Template Download” link under the “Flu Spreadsheet” group
- Choose “FluSafe 2012” from the dropdown box
- Click generate; the Excel spreadsheet will contain your employee’s names and birthdates
- Save this spreadsheet where you can access it throughout the flu season

First time and previous FluSafe participants:
After you’ve entered all of your employees* on the spreadsheet or updated your existing employees*, upload the file to MIIC.
Click on “Spreadsheet Upload” under the “Flu Spreadsheet” group, browse for your FluSafe spreadsheet, enter your phone number and click “upload.”
You must upload a spreadsheet with this information by December 31, 2012. |
PHASE 2
- Record shots using the same spreadsheet from Phase 1.
You will need to submit the following information for every flu vaccination:
• Vaccination Date
• Vaccine CPT Code
Leave these fields blank for employees who didn't get vaccinated.
- Upload spreadsheet with vaccination data in MIIC.
Use the same process as Phase 1.
We suggest uploading a spreadsheet once a month or when you have significant changes to your spreadsheet, an updated “FluSafe Summary” will appear the next day (MIIC updates overnight).
Final spreadsheet must be submitted by April 15, 2013.

* FluSafe’s definition of “employee”
For purposes of FluSafe, employees include all paid personnel in a health care setting including licensed independent practitioners.
Include employees who work in your facility for 30 days or more from October 1, 2012-March 31, 2013. |
