Register
- Register to participate in FluSafe. (closed December 1, 2011)
You and your secondary contact will receive a confirmation email with the following:

- How to contact your MIIC Regional Coordinator
- Link to the MIIC Quick Data Entry Spreadsheet
|
- Follow-up with your MIIC Regional Coordinator.
Ask questions, confirm participation, and make sure you have proper access to MIIC.

Phase 1
- Download the MIIC Quick Data Entry Spreadsheet. DO NOT change the format of this spreadsheet.
- Enter the information for ALL employees on the spreadsheet, you'll need the following information:

- First and last name
- Birthdate
|

To help eliminate duplicates, consider also entering: |
| |
- Middle name
- City, state, and zip code of residence
|
- Upload the spreadsheet with employee information in MIIC.
You must upload a spreadsheet in MIIC by December 31, 2011.

FluSafe's definition of "employee"
For purposes of FluSafe, employees include all paid personnel in a health care setting including licensed independent practitioners. |

Phase 2
- Record shots using the same spreadsheet from Phase 1.
You will need to submit the following information for every flu vaccination:

- Vaccination Date
- Vaccine CPT Code
|
| |
Leave these fields blank for employees who didn't get vaccinated. |
- Upload spreadsheet with shot data in MIIC.
Upload as often as you want, an updated "FluSafe Summary" will appear the next day (MIIC updates overnight).
Add new employees; do not delete employees who terminate after October 1, 2011.
Your final spreadsheet must be submitted by April 15, 2012.
