EvaluationWeb support for Internet Explorer Version 8 (IE8) is being phased out. IE8 does not support the database and display functionality that EvaluationWeb will require in the future. Luther Consulting recommends that users have IE Version 9 or later versions (IE10 or IE11), Firefox, Google Chrome, or Safari as their internet browser to access EvaluationWeb.

General Expectations:

  • Routine Matters: 2 Hours to Acknowledge and Return Email with Approximate Time Needed to Complete Issue
  • Create New Account: 7-10 Business Days to Complete once User Registration Form has been received
  • Secure Access Management Services (SAMS):
    If you've been waiting for more than ten business days and have not been contacted by SAMS, contact their help desk at samshelp@cdc.gov or 1-877-681-2901. If you continue to have difficulty, submit a help ticket below.
