EvaluationWeb: eAuthentication and Training Survey

In order to move MDH and CDC Direct Funded HIV Counseling and Testing Sites into EvaluationWeb, user names were submitted for the eAuthentication process.

We at MDH have some follow-up questions for users regarding the process.

  1. Download the Rules of Behavior: CDC System User Agreement (PDF: 240KB/15 pages) from CDC
  2. Read it
  3. Download and sign the "EvaluationWeb CDC Data System Users Agreements Between the Minnesota Department of Health - HIV and STD Section and MDH and CDC-direct Funded Agencies" (PDF: KB/1 page), and
  4. Fax only the signature page to Kathy Anderson at 651-201-4040.
