EvaluationWeb: eAuthentication and Training Survey

In order to move MDH and CDC Direct Funded HIV Counseling and Testing Sites into EvaluationWeb, user names were submitted for the eAuthentication process.

We at MDH have some follow-up questions for users regarding the process.

Have you been contacted by Secure Access Management Services (SAMS) via email regarding your eAuthentication for EvaluationWeb?

arrow If you answered NO to either of the above questions or have technical issues changing your Secure Access Management Services (SAMS) password or logging into EvaluationWeb, please contact Tina Klein via email.
