If you have specific questions regarding the Minnesota EvaluationWeb application, please contact the Minnesota EvaluationWeb Help Desk by completing the help desk ticket form or via e-mail at firstname.lastname@example.org, Monday through Friday, between 7:30 a.m. and 3:30 p.m., Central Standard Time.
EvaluationWeb support for Internet Explorer Version 8 (IE8) is being phased out. IE8 does not support the database and display functionality that EvaluationWeb will require in the future. Luther Consulting recommends that users have IE Version 9 or later versions (IE10 or IE11), Firefox, Google Chrome, or Safari as their internet browser to access EvaluationWeb.

Secure Access Management Services (SAMS):
If you’ve been waiting for more than ten business days after requesting e-authentication and have not been contacted by SAMS, contact their help desk at email@example.com or 1-877-681-2901. If you continue to have difficulty, submit an EvaluationWeb help desk ticket.

Luther Consulting is the developer of EvaluationWeb and administers the system at the national level on behalf of the Centers for Disease Control and Prevention (CDC).
