Reporting Form for Chickenpox and Shingles Cases in Schools
School data will help MDH determine trends, duration, and transmission patterns, and any association between cases and immunization or disease history.
Schools are to report all cases of chickenpox in students and staff and all cases of shingles in students.

If a beginning of an outbreak of chickenpox is suspected, schools should also call MDH at 651-201-5414 or 1-877-676-5414.
