What to report

Blastomycosis (Blastomyces dermatitidis) must be reported to MDH within one working day.

How to report Blastomycosis

  • Yellow Disease Report Card
    This disease may be reported using the MDH "Yellow Card (previously known as the "blue card")."

    • Blastomycosis Report Forms
      There are also supplemental reporting forms for blastomycosis. There is one form for reporting a human blastomycosis cases, and one form for canine cases.
