Required Annual Reports and Trainings

Each site that stores and/or administers Minnesota Vaccines for Children (MnVFC) vaccine must be enrolled in the MnVFC program and have its own personal identification number (PIN). Sites must renew their enrollment each year by completing the requirements below.

Separate stock sites

  • 2018 MnVFC Separate Stock Sites Online Training
    Required for the immunization manager and vaccine coordinator. Due Nov. 30, 2017.
    To view the online training use Internet Explorer 10, Firefox 4.x or later, Chrome 37.0 or later, or Safari 6.0 or later. If it doesn’t play in one browser, try another one. If the 2017 training plays, you will need to clear your cache. How to Clear Your Browser's Cache. The training contains audio.
  • 2018 Minnesota Vaccines for Children (MnVFC) Program Provider Agreement
    Complete and sign by November 30 of each year.
  • 2018 Minnesota Vaccines for Children (MnVFC) Program Annual Report of the Number of Immunized Pediatric Patients
    Complete and sign by November 30 of each year.
    If your site submits dose-level eligibility (DLE) information to MIIC, you can collect the count of pediatric patients using the "MnVFC Report" function in MIIC. See the MnVFC Reports in MIIC user guide on Vaccine Ordering and Management in MIIC.
