Influenza-like illness (ILI)

All schools:

Report when the number of students absent or sent home with ILI reaches 5% of the total school enrollment

Elementary schools only:

Report when three or more students from the same classroom are absent or sent home with ILI

Complete the Influenza-like Illness Report Form - Minnesota Schools 2010-11.

Chickenpox
(varicella)
  1. Notify MDH at 651-201-5414 or 877-676-5414 (toll free) when an outbreak begins. Definitions for an outbreak and when it ends are on the reporting form. Also, please notify your school district office.
  2. Complete the case-specific information on students and/or staff persons who develop chickenpox on the Chickenpox School Outbreak Report Form 2010-11.
  3. Fax to: 651-201-4820 when the outbreak ends.*
Shingles
(zoster)
All cases in students

Complete the Zoster (shingles) Case Report Form, Minnesota Schools, 2010-11.

Fax to: 651-201-4820*

Please notify your school district office.

Pertussis (whooping cough)
and other vaccine-preventable diseases including:
Measles
Mumps
Rubella
Diphtheria
Tetanus
Polio
Viral hepatitis
All suspected and confirmed cases in students and staff
