Ordering MDH Vaccine

There are three options for ordering MDH vaccine.

Option 1: MIIC
Order vaccine through the Minnesota Immunization Information Connection (MIIC)
a.	If you don't have a MIIC account, contact the MIIC Regional Coordinator that serves your region to get started.
b.	If you have a MIIC account start by completing the MDH Vaccine Ordering in MIIC Form. When that is complete you will receive further instructions about how to start ordering vaccine.

Option 2: Online
This online ordering system has been inactivated. Providers can order vaccine using MIIC or a paper form. To sign up for vaccine ordering in MIIC, go to “Option 1: MIIC” above and follow the appropriate instructions. More details can be found in the MnVFC Broadcast Fax from Dec. 3, 2012.
Option 3: Fax
Order vaccine by fax by printing and completing the appropriate form below.
