Conference Registration & Lodging Reservations
Attendees must register for educational components separately from meals and lodging.

|Conference Cancellation Policy|
Conference cancellation is separate from lodging/meals cancellation. To cancel your conference registration, please contact Liz Arita by phone (651-201-3880) or email (email@example.com). You will receive a complete refund if the cancellation request is received by August 17, 2014. Cancellations after August 17 will not receive a refund. Registration may be transferred to a substitute at any time by contacting Liz Arita by phone (651-201-3880) or email (firstname.lastname@example.org).
Conference registration is separate from lodging/meals reservation. Includes tuition and materials. Does not include meals, lodging, or travel expenses.

1. Download the lodging/meals reservation form above.
2. Fill out your registration information, and mail/fax/email to Cragun's, using the contact information on the form. The MDH block of rooms at Cragun's will be released on August 17; Cragun's will still take reservations based on availability.
|Cragun's Cancellation Policy|
Lodging/meals cancellation is separate from conference cancellation. You are responsible for your entire lodging and meals reservation. If you cancel or change your reservation less than 30 days prior to your arrival, there will be NO DEPOSIT REFUND. At 30 days or more prior, you will receive a REFUND less a $40 processing fee. When using a voucher or purchase order, cancellation/refund policy will apply. To cancel your lodging/meals reservation, please contact Lorie Bourassa with Cragun's by phone (218-825-2700 x 8400) or email (email@example.com). Reservations may be transferred to a substitute at any time at no additional charge, but those involved should notify Cragun's as soon as possible.
Lodging/meals reservation is separate from conference registration. $125 deposit required. Each roommate must complete an individual registration form, and forms should be submitted together. Attendees staying with family only need to complete one registration form. Each reservation will receive a confirmation of payment and receipt. Submit lodging/meals reservation forms by email (firstname.lastname@example.org), fax (218-825-1995), or mail (Cragun's Housing, 11000 Cragun's Drive, Brainerd, MN 56401).

Resource tables will not be staffed or used for sales or company marketing. Resource table opportunities are available only to local public health agencies, an organization in partnership with a local/tribal public health agency, the state health department, or a partner of public health. The Conference Planning Workgroup reserves the right to determine the appropriateness of resource table requests.
