REGISTER: Community Health Conference

The 2016 Community Health Conference will be held September 28, 29 and 30 at:

You must complete two steps to participate:

  1. Register for Conference with MDH
  2. Reserve Lodging and/or Meals with Breezy Point Conference Center

Step 1: Register for Conference with MDH

Use the MDH payment portal to register online as a conference participant. Cost is $70.00. Registration closes September 14, 2016.

MDH staff – please follow instructions provided on the MDH Intranet or contact your administrative support specialist.

Conference presenters – do not register with this link, please follow instructions provided via email.

Conference Registration Cancellation Policy

Conference registration cancellation is separate from lodging/meals reservation cancellation. To cancel your conference registration, please contact Liz Arita by phone (651-201-3890) or email (health.chconference@state.mn.us).

You will receive a complete refund if the cancellation request is received by September 14, 2016. Cancellations after September 14, 2016 will not receive a refund.

Registration may be transferred to a substitute at any time by contacting Liz Arita by phone (651-201-3890) or email (health.chconference@state.mn.us).


Step 2: Reserve Lodging and/or Meals with Breezy Point

Package reservation deadline is August 27, 2016. Reservations made after August 27, 2016 will be based on space availability at conference rates. Rooms will be assigned on a first-come basis. If you need meals only, you are encouraged to make your meal reservations prior to the event to aid event planning.

Consider sharing a cabin or condo with colleagues.

Three lodging/meals packages are offered as well as meal-only options.

How to Reserve Meals/Lodging

No reservations will be accepted without the fillable form: Breezy Point Meals/Lodging Reservation Form (PDF) Note - save the form to your computer to enter information electronically.

Full payment is due with the reservation. Credit cards will be debited for the full amount when the reservation is processed. If you must send a check or a copy of the purchase order separately, please indicate the name associated with the reservation. 

To share lodging with another conference participant, each roommate needs to complete the Breezy Point Meals/Lodging Reservation Form (PDF). Note: multi-bedroom units are single occupant per bedroom until all bedrooms are filled.

Conference participants may have non-conference guests stay with them for no additional cost; however, non-conference guests must make their own meal reservations if they wish to eat with conference participants.

You will receive an email confirmation from Breezy Point after your lodging/meal reservation has been processed.

Breezy Point Conference Center Cancellation Policy

Lodging/meals reservation cancellation is separate from conference registration cancellation. You are responsible for your entire lodging and meals reservation. If you cancel or change your reservation less than 30 days prior to your arrival, there will be NO DEPOSIT REFUND. At 30 days or more prior, you will receive a REFUND less a $25 processing fee. When using a voucher or purchase order, cancellation/refund policy will apply.

To cancel your lodging/meals reservation, please contact Mary via email groups@breezypointresort.com. Reservations may be transferred to a substitute at any time at no additional charge, but those involved should notify Breezy Point as soon as possible.
