Conference Registration and
Lodging/Meals Reservations

The Community Health Conference will be held October 7-9, 2015 at:

You must complete two steps to participate:

  2. Reserve Lodging and/or Meals

The registration link above will take you to the MDH payment portal. Follow the steps to register online for the conference. Cost is $52.00. Registration closes September 30, 2015.

Conference Cancellation Policy

Conference registration cancellation is separate from lodging/meals reservation cancellation. To cancel your conference registration, please contact Liz Arita by phone (651-201-3880) or email ( You will receive a complete refund if the cancellation request is received by September 15, 2015. Cancellations after September 15 will not receive a refund. Registration may be transferred to a substitute at any time by contacting Liz Arita by phone (651-201-3880) or email (

The block of rooms for this conference will be released after September 7, 2015. Reservations made after September 7, 2015 are subject to Cragun's availability.

Cragun's Cancellation Policy

Lodging/meals reservation cancellation is separate from conference registration cancellation. You are responsible for your entire lodging and meals reservation. If you cancel or change your reservation less than 30 days prior to your arrival, there will be NO DEPOSIT REFUND. At 30 days or more prior, you will receive a REFUND less a $40 processing fee. When using a voucher or purchase order, cancellation/refund policy will apply.
