2010-2014 Assessment and Planning Cycle
In 2015, Minnesota community health boards submitted the following deliverables as part of a five-year assessment and planning cycle:
- Community Health Assessment (Deliverable: 10 Most Important Community Issues)
- Community Health Improvement Plan
- Organizational Self-Assessment (Deliverable: Three Standards Most in Need of Improvement)
- Strategic Plan
- Quality Improvement Plan
Community Health Assessment (Deliverable: 10 Most Important Community Issues)
A community health assessment provides the foundation for improving and promoting the health of the community; it identifies and describes: (1) the health status of the community, (2) the factors that contribute to health challenges, and (3) existing community assets and resources that the community can mobilize to improve the health status of its residents.
In Spring 2015, Minnesota's 48 community health boards reported the 10 most important community health issues, identified through their community health assessment.
While MDH did not require community health boards to submit their community health assessments, some community health boards have opted to post their community health assessment on their own websites. Community Health Improvement Plan
A community health improvement plan is a long-term, systematic plan to address issues identified in the community health assessment.
The purpose of the community health improvement plan is to describe how the health department and the community it serves will work together to improve the health of its population.
Organizational Self-Assessment (Deliverable: Three Standards Most in Need of Improvement)
While developed for the purpose of accreditation, the national public health standards developed by the Public Health Accreditation Board (version 1.5) allow local, state, and tribal health departments to assess themselves consistently, and identify strengths and areas for improvement.
The three standards identified by Minnesota's community health boards can be used locally to inform quality improvement plans and strategic plans. At the state level, the standards are used to inform opportunities for improvement, as well as training and technical assistance needs.
A strategic plan is an internal document that sets forth what an organization plans to achieve, how it will achieve it, and how it will know if it has been achieved. A health department's strategic plan focuses on the entire health department.

Quality Improvement Plan
A quality improvement (QI) plan is an internal document that describes how the organization will manage, deploy, and review quality throughout the organization. It discusses how quality improvement approaches, methods, tools, and techniques, as well as application of lessons learned from evaluation will be used throughout the organization.

If you are interested in obtaining your community health board's QI plan, please contact your community health board directly. Community health boards are not required to submit QI plans as a deliverable for the 2015-2019 Assessment and Planning cycle.
